How to Do a RACI Matrix Using a Microsoft Word Template

A Microsoft Word RACI template is a project matrix that shows who is responsible, accountable, consulted, and informed for each project task. Use a RACI template to clarify project roles and responsibilities among team members.  

RACI matrices help project teams avoid confusion about pending project tasks and to oversee tasks assigned to team members outside the department. Whereas a project timeline enables you to add tasks, task owners, due dates, and statuses, a RACI matrix shows all the key individuals involved in the completion of a task. 

RACI is an acronym for responsible, accountable, consulted, and informed. Consult this list when deciding which letter to assign each team member:  

  • Responsible: Assign an R to the team member who is responsible for completing the task. 
  • Accountable: Assign an A to the individual who is accountable for ensuring the task is completed by the assignee.  
  • Consulted: Assign a C to the individual who has been consulted on the task. 
  • Informed: Assign an I to the individual who has been informed that the task has been created. 

While individual templates may vary, most RACI templates for Microsoft Word typically include the following components: 

  • Project Title: Provide the name of the project to which you will apply RACI matrix attributes. 
  • Project Manager: Enter the name of the project manager responsible for ensuring the project is completed.  
  • Phase: Enter the project phase under which you will assign each task and RACI matrix component. 
  • Task Description: Provide a brief summary for each task that must be completed. 
  • RACI: Designate which team members or interdepartmental stakeholders are responsible, accountable, consulted, and informed for each task, per the RACI matrix
  • Percentage Complete: Provide a percentage of completion for each task.
  • Due Date: Set a due date by which the task or deliverable must be completed. 
  • Status: Include the status of the task so that team members and other stakeholders are aware of how close it is to completion. 
  • Notes: Add any relevant notes related to RACI matrix roles and responsibilities for each project task or deliverable.

RACI Templates for ITIL

RACI templates are also helpful to IT departments using the Information Technology Infrastructure Library (ITIL). ITIL is a set of practices for IT service management (ITSM), IT change management (ITCM), and IT incident management. ITIL RACI templates apply RACI matrix attributes to ITIL efforts, so that it’s clear to all IT personnel and stakeholders which IT team members are responsible, accountable, consulted, and informed for any given IT task.
